SQL query to get transfer history coulmn wise(horizontally)?

When a person has two or more location transfers applied on him, I am looking for a sample query to get data in below format. Please help if anyone has such query:
Person Name| Person Number| 1st Location transferred from | 1st location transferred to| 2nd Location transferred from | 2nd location transferred to
