Oracle APEX New Account - Invalid Login

We are using Oracle APEX and am adding a user account. All settings are the same as other administrators and yet the login is always invalid and now locked. Yet, under user account management does not show it as locked.

I have tried deleting and recreating the account name using a generic name and email address. I have also simplified the passwords to match the same number and types of characters. Same issue. Any thoughts?
