Manage File Import Activity - While updating Address for Branch - Getting status as Unsuccesful

I am trying to import Address details for Bank Branches and importing the .CSV file which consists of Address (Address Line 1,State, City,Country).

Here am getting the status as Unsuccessful.

Please assist me whats the issue and what are the corrections which i need to consider.

Steps i had followed:

  1. Prepared .CSV File

  2. Manage File Import activities --> create New -->
  3. selected file to import from Desktop
  4. Mapping i had selected as below

Finally am getting the below status which is not completing the process
