How to insert only unique records in Apex

Hi All,
I am having following table in Oracle :
CREATE TABLE T_REDL_USECASE_TABLE_ITSP(
PKGID VARCHAR2(20), --SEQ
UCNO VARCHAR2(20), --SEQ
USERID VARCHAR2(10),
SELECTED_TABLE VARCHAR2(20),
SOURCE_SYSTEM VARCHAR2(20),
DOMAIN_NAME VARCHAR2(20),
STATUS VARCHAR2(20),
NAME VARCHAR2(20)
)

UCNO, PKGID together are primary composite key.
every time for the given UCNO, PKGID user can enter multiple tables, and these should be stored against given UCNO, PKGID.
I created sequenceno for PKGID and UCNO, it's inserting every time new records.
Can you please tell me how to achieve this? (Ex; shooping cart application)
