XML-20121: (Fatal Error) End tag does not match start tag 'StatusCode'. could anyone help here?

This is a XML (using PL/SQL) report output as PDF. It has been running successfully around 7 years now without any errors.

Currently we are facing "XML-20121: (Fatal Error)" for some persons,

And it also works for some person record successfully.

Hence could anyone of you please suggest how to go further?
