Workflow mail not being sent for one user only

we are having issue with one user every now and then and unable to figure out the root cause of this problem. All the other users get emails fine for PO Approval except this one. In the wf_notifications table, the mail_status = null and status = CLOSED. There was a workflow mailer restart but not sure how it can impact only one same user. I checked her email address is fine. What could be a permanent fix here.
