why slow DML in Index organized tables

I have learnt that IOT scheme is not suited for tables having high volume of DML operations.

I want to know that:

1.) Why DML operations are slow when we have index and data at the same place for IOT?
2.) why we take extra care for fragementation in IOT than heap organized tables?
