When I fill a form field using a SQL Query the result is displayed but not saved in the table.

As I write in the subject, I created a form and in some of their fields I have used a SQL Query in order to get information of a table depending the user who was logging on.

The information is well displayed, however when I press "Send button" the information loses and the table field is showed empty.

Does somebody know what's going on?

I attach you bellow a picture of the problem
