What can cause Wasted space on indexes

From the EM manager console, I found that a table showing the following stats:
Table X: Space allocated 10M, space used: 9M, wasted space 0.1%
Index_X: Space allocated 12M, space used: 7M, Wasted space 40%
When I deleted the index_X, and recreated it, the above stats on the index_X did change !!,
My question:
What can cause an index to have a huge wasted space even though I recreated it?
