What can cause "ORA-25408: can not safely replay call" with all instances in RAC running ok ?

Hello,

I am getting "ORA-25408: can not safely replay call" using DRCP pooled connections on a 2 nodes RAC SE ( 12c )

Connections are on a service properly configured for TAF , but when I look at instance activity I can see that both instances are up since months ... no failover should have occurred.

Do you know in what cases connections could get the ORA-25408 even if instaces keep running smoothly ?
