What are the reason for getting error 'invalid file operation'

What are the reason to get the error :-

ORA-29283: invalid file operation
ORA-06512: at "SYS.UTL_FILE", line 449 ORA-29283: invalid file operation

And the DB user has the read/write access on the unix directory.
