Webcenter Portal / Content upgrade from 12.1.0.0.* to 12.2.1.3.0

Gurus,

We need to upgrade Webcenter Portal / Content from 12.1.0.0.* to 12.2.1.3.0. I am wondering if we need to install all components WebLogic, WebCenter Portal and WebCenter Content and run upgrade agent. Or Is there a bundle patch to accomplish the same.

If we are fine with BP, can you point me to the documentation as well.
