web application image fa-undo-alt issue

Hi All,

 Using APEX 5.1.1 with universal theme. I am referencing fa-undo-alt in Icon CSS classes in my APEX application region, but the image is not detected in my region's button. However, on the same page it shows up under the Actions menu dropdown.

Please advise if there is something I am missing in my install. See image below.
