upload csv data

hi
I want to upload my csv file into table without using utility function...So I refered vikas sample application in the following link..

I can run the packages and i made a trial of using this application also...the report is generated but the data is not stored into the table when I give table name in the textbox...It produces the' missing or invalid option' error...Can anyone make a trial of this application and tell me why this error is coming? For the past 2 weeks I am trying this program..But could not get a solution...please give me a suggestion...
