Unable to open Oracle Forms 12c Migration assistant in windows.Please need step by step guidance

Want to migrate Forms 6i to Forms 12c with FMA. But unable to start the wizard in windows.

After setting the path SET.FORMS_PATH = %ORACLE_HOME%\forms in the command line when frmplsqlconv.bat mode = wizard is given . The wizard is not opening .

Please help?
