UCM Web Form File Uploads

Any ideas on how to implement ability to upload files in a UCM web Form created in a SiteStudio website?

Also when i try to turn on enctype="multipart/form-data" for my ucm form it seems to break the service SUBMIT_HTML_FORM i get an error "Failed to locate document information for document with content ID 'idcplg'. The document is no longer in the system". Not sure what is causing this?
