I have been working with open epic fhir.epic.com for a while with the Eureka platform at UCSF that hosts research studies. In the platform we use a patient facing SMART on FHIR application that "automatically" downloads client IDs to available epic providers. This is important for us as there are 400+ epic providers that we want to allow study participants to connect to and to grant us oauth tokens for access to their EHR data. If we had to negotiate a manual process with each provider to enable our SMART on FHIR app the cost in time would be unreasonable.

Do you have something similar at Cerner? I could not find anything in the cerner documentation. In https://fhir.cerner.com/authorization/, it says "As a registered client, Cerner organizations may then ask for your client application to be enabled, which is necessary in order to gain access to their protected resources.". This suggests that you have to somehow contact each cerner organization directly (email?) and request that your app be downloaded.
