SQL Output With Hyperlink

Hello,
I would like to have SQL output with hyperlink along with columns, expected output is below:

image.pngDownload link is dynamic, upon clicking the link, user should be able to view or download attachment.
How can I achieve this? Report will be in excel format; sample data with expected output is uploaded.
Appreciate any insight or help
