Some strange issues-Need solutions

1) I've created an user test and gave permission to BI author and consumer.Now Im able to see Home, catalog,Dashboard and everything in Presentation service.
I went to console and deleted the user test and again recreated with the same groups added to it.
Now when I log in im able to see Home, catalog,Dashboard and everything in Presentation service.But after some time catalog folder is gone.This is really strange.
2) The first time Im able to set My Dashboard as the default in My account.But second time (after deletion and recreation of same login) Im not able to set My Dashboard.It gives error

3) After sometime for some reason I restarted the services and again logged in using test used.It throwed error saying authentication failed.
Please help how to solve these.Am I doing any mistake? What is the proper way of assigning permissions?
