Simple Yes/No confirm modal popup window

Hi,

I would like to get a simple yes/no confirm modal popup window. It should be launched when user press on a "delete" button in a form.

This should launch the popup, if user press "No" popup should be dismissed, if user press "Yes" a PL/SQL function body should be launched to delete the item effectively from database.
