Sim toolkit and JavaCard

HI company

I have just posted a thread "New to java card" to ask about some basics in java card development,and thatnks to every one answers me.
Now after I read that java card 2.2.2 and java card 3 connected edition can can deploy applications ( applets,servlets) to a Sim card (java card 3 supports sim card new generation),now I wanna see how can I develope an applet (of java card 2.2.2) for a sim card and how can I read info deploy it into the sim card,could I use cellphone to deploy it or I should us PCSC? and if anyone could help me to show me how could we develope an entire Sim toolkit ( applet for sim card ),it will be good for me and for any new java card developer.
