Apex saved data without mandatory fields inputs

Apex 23.1

here is a Form which marked some fields as Required but it saves Form without data in mandatory fields, what can be the cause of this problem? have a look at screenshot below. page submit process is default Automatic Row Processing (DML) , please help.
