root.sh fails (OEM GI provisioning) with PROC-26: Error while accessing the physical storage Storage

While provisioning oracle RAC GI 12.1.0.2 from OEM on a two nodes, I encountered the following error.

=>. The root.sh on one of the nodes it fails with the following error. <=

PROT-1: Failed to initialize ocrconfig

PROC-26: Error while accessing the physical storage Storage layer error [Insufficient quorum to open OCR devices] [0]

ASM was up and running on that node where the root.sh script failed.

There was one diskgroup CRS_GRP mounted and it has spfile and password file for ASM existing.

ocrcheck fails with same error.

I am not able to figure out what it exactly means by Insufficient quorum.

The CRS_GRP diskgroup has 3 x 10 GB luns with normal redundancy. 30GB I think is good enough for a DG that has ocr,votingdisk.

The cssd logs show the ocrcofing failed with above error.

Any assistance is greatly appreciated.
