REQAPPRV (PO Requisition Approval) Workflow Notification

Hello gurus! I am hoping someone could help me on this.

My company have PO Requisition Approva Notifications being sent out to users to approve.
On some of the email notifications the requisition line detail shows project code/name/task. If I go in the apps to Sysadmin > Administrator Workflow > Status Monitor and look up the notification message the project info is there.

Has anyone encountered this problem? We are using 11.5.10.
