Refresh tabular form region using dynamic action

I am stumped on this scenario please advise. I have a dynamic action on a button that when clicked fires a true pl/sql action that will add a member to a collection this is working as expected, however the second true action on the same button is going to refresh the tabular form/report region. This is not refreshing my form/report.
To find out if indeed it is possible I created a report on the same page based on the same view and the dynamic action works.
So why does the refresh dynamic action work on my new report and not on the tabular form. I thought a tabular form was just an updateble report.
I am using apex 4.1.0.00.32
Please help.
