redirect to default page when use task flow on any button click

I am using jdeveloper 12.2.1.4 and use a bounded task flow. I am facing the issue that after open any page and when click on 'add new' or 'save' button it redirect to default page. same issue come on all pages.But it work ok without task flow. please guide me what is issue. my task flow attached
