Receiving Approve/Reject Emails with Attachments

In CRM as part of the standard functionality when a service request is sent as an email for approval to a person, he/she can approve or reject it. I have a customer who would like to give the person the additional option of attaching a file when approving or rejecting the service request through email. This file should then be attached to the service request.
Has any of you done something similar before ? Any help in this regard will be highly appreciated.
