Random HTTP 408 error encountered

I'm having random HTTP 408 error in my Sun One 7 access log. The problem happens randomly in any functions. Any idea where does the the problem lies? Is it in the web server, web agent or policy server? Thanks.

My configuration is as of the below:

Web Server: Sun One 7 Update 6
App Server: BEA Weblogic 8.1 SP 6
Siteminder Policy Server: 6 SP5
Siteminder Webagent: 6QMR5
