Put Text on top of Shuttle

I'm hoping someone can help me with some javascript here - I have been asked to label the shuttle like (Available Columns + Selected Columns). Now since the shuttle object is a compound object, it has to be done with JS. Problem is I am not very good at it and I've spent hours just getting the labels UNDER the box. I know I'm close, but was wondering if a kind soul wants to help me out. Basically just looking for a little nudge in how to insert a new row above the shuttle row that Apex generates.
