Problems with Forms_ddl insert

Hello,
I am trying to insert a record in a temporary table using Forms_ddl but the record does not get inserted. When I try to run the same insert command outside in SQL Plus it seems to work fine. Can youi please advise what the problem could be.
A similar thread is posted here and I have tried all their suggestions but nothing seems to help, please advise.
