Print checkboxes in pdf using XSL-FO

I've created a report based on a query that contains a checkbox using apex_item.checkbox. I'd like to print this report to pdf, but the checkbox-column is empty. The application is on a hosted environment and uses XSL-FO. BI Publisher cannot be used.

What I'd like to have is checked and unchecked checkboxes in a pdf-report based on XSL-FO. I've searched the internet, but cannot find an example how to do this.

Can somebody help me with this?
