Patch set for Clusterware 10.2.0.4

Hello Experts,

We have a two node RAC database win 2003.
now we are planning to upgrade 10.2.0.3 Oracle clusterware and database to 10.2.0.4 .

we have a seperate upgrade patch for clusterware and database or clusterware and database includes in the patch?

Also it will be really nice if we could get some docs to complete this activity .
