Oracle Workflow 24.2 - Human Task (Approval & Action) - Faulted state

Hi,

 I had created one workflow for multi-level approvals with both approval & action tasks.

If approver rejected request, then it will back to requestor(initiator) for action task.

In that everything working fine but out of 100 or 200 workflows 1 workflow goes to FAULTED state.

If i do RE-RETRY the workflow with admin login then it goes to ACTIVE state without any changes.

Not sure what i had missed.

Really appreciate if anyone can help on this.
