Oracle VM 2.2.1 VLAN and Cisco Switch

Hi,

I'm trying to configure VLAN access on my Oracle VM 2.2.1 server. When I configure bonding for eth0 and eth1, it works fine. However, when I configure VLAN on the server I lose network connectivity and my switch port often goes into "err disabled".
I have worked with Oracle Support on my VM configuration, and it seems to be OK. The notes followed from Oracle Support are:
Configuration of Network (NIC) Bonding in Oracle VM for x86 [ID 738269.1]
and
Enabling Bonded Virtual LANs (VLAN) in Oracle VM [ID 746103.1]

Oracle support says it is a problem with my switch configuration, so I'm wondering if anyone has any insight into how to configure a Cisco Catalyst 3012 correctly for VLAN use on Oracle VM.

This is a snippet of my switch config: (port 0/4 is the one i'm working on)
