Oracle Migration Workbench -MS Access97

I have used Oracle Migration Workbench (OMWB) to migrate a huge MS Access 97 database to Oracle Database. The Migration Work Bench worked fine. I have to remove "Relationship","Referential Integraty" and "Cascade UPdate/Delete" functionality from relationship diagram before migrating without any errors.

Most of the forms and dat entry are working fine except one form. This form is opening like the MS Access application but the data entry part
fields are not showing up at all. That is no data entry fields are showing up.

Can any one help me to trouble shoot this problem]
