Oracle forms - Inconsistent save issue

Hi,

I have an oracle forms application running on Oracle Application Server 10g Version and uses 10g oracle database as well.
I have a form where a record is mapped to table in database.
When I enter data in all the fields and click on save, data in last field is getting saved as blank.
This is not happening everytime, most of the time data getting saved properly on second try.

This issue is disappearing once we do application server and database server restart but this issue again pops up after a week or two.

Can someone let me know what could be the possible reason for this issue?
