Oracle FMW, Form not able to run through builder and http://localhost:9001/forms/frmservlet not lau

Regarding System and S/W specification

=======================================

System OS - Windows 8.1 Enterprise (64-bit Operating System, x64-based processor)

Oracle DB - 12.2.0.1

FMW Infra - 12.2.1.3.0

FMW Forms and Reports - 12.2.1.3.0

Client Side Java - 1.8.0_141

I have installed Weblogic infra and then installed forms and report. Had done RCU setup and configuration. All servers are started. here is Weblogic server Administration console.

Enterprise Manager Fusion Middleware control 12c

ohs1 server is running. I did all CLASSPATH, FORM_BUILDER_CLASSPATH, etc... value setup, But Again when i run my form builder got following error message.

Did Setup of forms parameter, but still not resolved.

1. How to resolve this?

Regarding Builder

Web Browser Location value C:\Program Files\Internet Explorer\iexplore.exe

Application Server URL value - http://<Machine Name>:9001/forms/frmservlet

2. when I run the 'http://localhost:9001/forms/frmservlet at IE, I got following error message. "Internet Explorer blocked this website from installing an ActiveX control" with 'Install' option. If I press install button got

Please help me with these 1 and 2 questions.
