Oracle EBS Workflow - Not showing Action History

Hi Folks,
I have developed a custom workflow in Oracle EBS(12.2.9) and it's working fine except showing the Action History,
due to this, second level managers were not able see who approved at the first level,
could you please share some insights to fix this issue.
