Oracle APEX jquery Slider

Hello All,

Anyone please help me out with implementing jquery slider something like this.

I just need a default one where users can select a value from 0 to 100% using the slider. I am using Oracle 11g r2 database and oracle apex 4.2.1 version. I also need to store this value in the database when user submits the page; also need to retrieve this value from the database once user select this record.

I tried to search the oracle apex forum and found the below link of Jari; but when I tried to implement the same thing I was not able to do so. I am not able to understand whether I am doing it correctly or not.

jQuery UI Slider: unable to enable

I have setup an example in my personal workspace. In my sample application; I just want to change the field satisfies Def (%) in the form to a jquery slider. If anyone can please have a look that would be great and I really appreicate it. Below are my workspace details.
