We had recurrent ORA-7445 errors on Database Server Oracle 12.1.0.2 (O.S : RHEL 7.3)
The stack trace shows :

and to unresolved Bug 22174441 : ORA-07445[LXOCNTBYTE]/ORA-07445[LXKTRIM] INTERMITTENTLY ON SELECT SQL
On our platform "Redhat 7.3 + Oracle12.1.0.2", setting parameter : OPTIMIZER_ADAPTIVE_FEATURES to FALSE
seems to be a correct workaround solution.
