ORA 54 Ghost error message - Internal Error?

I have come accross two situations now where our application (multi threaded) has been getting Oracle 54 error messages. From a developer point of view I was 100% sure that we were not trying to update the same rows. Both times re building the table in question in Oracle solved the problem. The issue is that this has now happened twice to me in two different companies with different applications. Does anyone know what the cause of this problem is? The table in question has 6 million rows in it and is updated all day. It has a number of indexes on it too. is the ORA 54 some kind of last ditch error message when oracle doesnt know what else to do?
Its very bad as when you ask a DBA for help they just say that its an application issue as oracle says we are causing locks. In fact we are not.

Any help would be great.
