ORA-15080: synchronous I/O operation to a disk failed

I guys,

Fresh install of two node RAC 11g R1 with 11.1.0.7 patch and PSU patch (11.1.0.7.1) as well.

OS is OEL5 U6

Trying to create disk groups, but keep getting these errors "ORA-15080: synchronous I/O operation to a disk failed"

ASM ALery logs says:
ORA-27070: async read/write failed
WARNING: IO Failed. subsys:System dg:0, diskname:/dev/X0R disk:0x1.0xe94f3ea1 au:0
iop:0x2b05ebe28880 bufp:0x2b05ebd87400 offset(bytes):0 iosz:4096 operation:1(Read) synchronous:0

Errors in file /u01/app/oracle/diag/asm/+asm/+ASM1/trace/+ASM1_ora_12568.trc:

Any ideas?

I have read the article :

But it seems the issue is fixed in 11.1.0.7, thats the release i am on.
