ORA 1403, Error: Failed to close Oracle connection

Hi,

I am trying to get data out of an Oracle database using an application and am recieving:

Error ORA 1403
Error: Failed to close Oracle connection

Any suggestions as to what could cause this? If not any way I can see a history of SQL commands that have been used which might have caused this error? It is Oracle 10g on Win2003. Thanks in advance.
