Ora-12518: TNS:listener could not hand off client connection

We get the following error on our Oracle XE database:
Ora-12518: TNS:listener could not hand off client connection

We have increaseed the processes parameter to 200 but still get this error.

We eventually have to 'bounce' the database as no more sessions can connect.
