ORA-12516, TNS:listener could not find available handler with matching protocol stack

DB:11.2.0.4

JDBC thin client application getting intermittent ORA-12516 error and getting resolved automatically. Application using SCAN to connect to the database. We are working with oracle in a sev1 SR and they suggested to upgrade the JDBC driver to 11.2.0.4 version and apply the patch 17284368 on top of it. We still got the same error. Then oracle suggested to upgrade the JDBC driver to 12c. Still the same issue persist.Any help is much appreciated.

Connection pool min connections 50 and max connections 400
