ora-06531 reference to uninitialized collection while users try changing password

i have created a profile for the users enforcing password policy and then set their users to expires in order to allow them changing their password through toad or SQLPLUS , but when a user try to write a new password they get error ORA-06531 and password remains unchanged,please any help
