ORA-06413 connection not open

Hi all,
the client system is having the windows 7 64-bit operating system and the server is windows 2003 32-bit operating system and oracle 91 databse. while i am trying to connect the server from client system through sqltools, i am getting the error ORA-06413 error. and also i installed oracle forms and reports 6i. i can able to run my project using the same server and database in client system. i am facing the error in connecting the database through sql tools
any body can help me how to resolve this error
