ORA-00001: unique constraint (.) violated in a simple cursor

I have been receiving "ORA-00001: unique constraint (.) violated" error in a Simple Cursor.
This exception is not consistent.

There is a job which gets triggered every minute and calls a routine say Package.P1.

We are using 10.2.0.2 environment.

Has any one of you experience similar issue?
