ORA-0 followed by ORA-04030

In recent times the frequency of this error is on the rise. Am using Release 10.2.0.1.0 - Production Starndard version with SGA 1496 MB and PGA 998 MB, running Windows Server 2003

ORA-04030: out of process memory when trying to allocate 123404 bytes (QERHJ hash-joi,kllcqas:kllsltba)
The error always appears in 'SYS$BACKGROUND' process, and once in a while oracle gets terminated after giving 'Unable to begin another thread.' error and 'Oracle service is terminated' log in event viewer.
Can anybody give the cause of it and remedy to avoid it?
