Open Concurrent Request View form from Custom Form

Hi,

I am trying to SUBMIT REQUEST from custom form and call Concurrent Request view form then close the current open custom form.

Right now i am able to trigger the request from custom form and show the user about the request ID but unable to open concurrent requests view window to show the progress of the request to user and also unable to close the current window.

Please let me know if some one is aware of the solution.
